CHEST PA VIEW
ANATOMY
chest x-ray body & thoracic part में present organ की radiograph लेने के लिए किया जाता हैं। thoracic cavity में अपनी body के कुछ मुख्य organ स्थित होते हैं, जैसे heart, lungs, trachea esophagus, aorta आदि। इस सब की सुरक्षा के लिए thoracic cage होता हैं,जिसमे anteriorly sternum, posteriorly vertebral column तथा laterally ribs स्थित होती हैं। Superiorly thoracic inlet and inferiorly diaphragm होता है, जो thoracic organ को protect and करता हैं ।
INDICATION
- Chest pain
- Fever
Persistent Cough
✓Tuberculosis
✓ Shortness of Breath
✓Broken Ribs
✓Emphysema
✓Heart Failure
✓Lung Cancer
✓Pneumonia
✓Pneumothorax
✓Pleural Effusion etc.
CONTRA INDICATION -
Follow ten days rule in female
PREPARATION OF PATIENT FOR CHEST X RAY
Chest x-ray से पहले Patient के Jewellry, Eyeglasses, Body piercings तथा दूसरे सभी metallic objects हटा देना चाहिए ।
Patient में कोई surgical implanted device जैसे pacemaker या heart valve आदि हो तो सबसे पहले doctor को बता देना चाहिए । इस स्थिति में doctor chest x-ray के दूसरे किसी विकल्प का विचार कर सकता हैं जैसे CT Scan या Sonography आदि x-ray से पहले Patient को अपनी waist से ऊपर के कपड़ो को हटा कर hospital gown पहनने को कहा जाता हैं।
POSITION OF PATIENT
एक 35 X 35 cm की cassette या Patient की size के अनुसार ली जाती हैं। Patient को x-ray cassette की और मुँह करके इस तरह खड़ा करते हैं की Patient की ठोड़ी (Chin) cassette के center | Patient Median sagittal plane a cassette plane parallel रखते हैं। shoulder को थोड़ा आगे की ओर मोड़ा (Rotated forward) तथा थोड़ा निचे की और दबाया (Pressed Downwards ) जाता है। इसके लिए hands के Dorsal aspect को hip की निचे की और रखा जाता हैं जिस से scapula lung field से अलग किया जाता है
Direction and Position of X-ray Beam- collimated तथा horizontal x-ray beam काम में ली जाती हैं। x-ray beam को image receptor या cassette से parallel (right angle) पर रखा जाता हैं तथा x-ray beam का center 7th thoracic vertebra के level पर रखा जाता हैं। exposer full normal arrested inspiration condition में किया जाता हैं ।
IMPORTANT POINTS
1. एक ideal Chest PA Radiograph में निम्न विशेषताये होती हैं-।
x-ray film में lung field पूरा आता है तथा scapula lung field से पूरा अलग हो जाती हैं। x-ray film पर body anatomy में कोई rotation नहीं होना चाहिए। इसके लिए anterior ribs end देखे जाते हैं जो दोनों ओर vertebral column से सामान distance पर होने चाहिए। दोनो clavicle bone एक दुसरे के सामने होनी चाहिए, lungs अच्छी तरह से फुले होने चाहिए इसके लिए ये देखा जाता है की lung field में anteriorly 6 (six)ribs तथा posteriorly 10(ten) ribs दिखनी चाहिए । inferiorly costophrenic angle (CP Angle) तथा diaphragm की outline clear दिखनी चाहिए।
2. Pneumothorax की उपस्थिति देखने के लिए full Expiration में radiograph लिया जाता हैं। इस स्थिति में Intrapleural pressure बढ़ जाता है जिससे Pneumothorax बड़ा दिखता हैं।
