HYSTEROSALPINGOGRAPHY | HSG
HSG fallopian tube और uterus की एक विशेष प्रकार की X-ray procedure होता है। यह procedure लगभग 45 मिनट का होता है।
Indication :-
✓ Infertility- Infertility इस procedure का मुख्य कारण होता है ।
✓Recurrent miscarriages बार बार होने वाला गर्भपात|
✓Following tubal surgery- fallopian tube surgery a tube opening at check करने के लिए|
✓Assessment of the integrity of a caesarean uterine scar- cesarean scar at check करने को।
✓tumor या अन्य inflamation देखने के लिए
Contraindications
✓Pregnancy- Ten Day's Rule का पालन किया जाता है ।
✓Patient के vulva या cervix के inspection के दौरान अगर कोई purulent discharge दिखे या पिछले 6 महीनो से Patient को PID हो तो यह procedure नहीं किया जाता है।
✓अगर Patient lodinated contrast media के प्रति sensitivity हो तो भी यह procedure नहीं किया जाता है ।
Equipment
Patient preparation
✓Patient को appointment तथा examination के दौरान sexual intercourse से दूर रहना चाहिये | या contra septic method का use करना चाहिए। ✓अगर किसी महिला के Menstrual cycle नियमित है तो अर्थात 28 दिन का है तो उसे procedure लिए 4th से 10th दिन तक appointment दिया जा सकता है।
Preliminary Film
pelvis cavity का collimated view लिया जा सकता है ।
Patient position
Patient को examination table पर supine लिटाते है | Patient के knee joint को flex देते है तथ legs को दूर ( abducted) कर देते है, तथा heel को एक दूसरे के पास रखते है ।
Aseptic technique का use करते हुए operator vagina में speculum डालता है तथा vagina व cervix को chlorhexidine से क्लीन क्लीन किया जाता है । Cervix के anterior lip को vulsellum forceps से पकड़ कर cannula को अंदर डाला जाता है । Catheter में syringe की help से contrast media अंदर डाला जाता है। ये ध्यान रखा जाता है की Syringe तथा cannula में air bubble न रह जाये ।
Films
जैसे ही fallopian tube में contrast media भरना शुरू होता है तो image लेते है। कम से कम 4 spot film ली जाती हैं ।

Tubal filling phase
Peritoneal spillage दिखाई देते ही अगली film लेते हैं ।
AFTER PROCESSOR
यह सुनिश्चित किया जाता है की Patient discharge से पहले एकदम आरामदायक अवस्था में हो तथा
ज्यादा bleeding न हो रही हो ।
Patient को यह सलाह दी जाती है की उसे अगले 2-3 दिन तक vagina से bleeding हो सकती है जो 2 week तक रह सकती है।
